Riverside Health System Adopts Phytel Transition(TM) for Post Discharge Follow-Up

New Automation Tool Helps Contact Discharged Patients and Identify Those Patients at Risk for Unnecessary Readmission, While Bolstering Patient Satisfaction Scores (March 27, 2012)

DALLAS, TX -- (Marketwire) -- 03/27/12 -- Phytel, the leader in physician-led population health improvement, announced today that the Riverside Health System in Newport News, Va., has implemented the Phytel Transition™ solution in the emergency department at Riverside Regional Medical Center to provide better service and monitoring of patients who have been discharged from the healthcare system. Riverside plans to extend the Transition product to other areas of its integrated delivery system.

The 350-physician Riverside Medical Group has been using the Phytel Registry and Patient Outreach solution for nearly three years to help its primary care providers address gaps in preventive and chronic care. The positive financial and clinical impact of that program was one factor in Riverside Health System's decision to select the Transition product to improve the patient experience.

Recognizing that patient care transitions after discharge can affect not only patients' health, but also publicly reported patient satisfaction scores, Riverside Health System views Phytel's automated solution as the best way to communicate with all of its discharged patients. With Phytel Transition, 100 percent of patients are contacted within 72 hours of discharge providing an effective and efficient way to remain connected with patients.

The automated message from Riverside asks patients a series of questions, including whether they would like someone from the hospital to call them and whether they would recommend the hospital to a friend or relative. A Riverside nurse calls back patients who need additional assistance or have questions about their discharge instructions.
