Phytel Showcased at Health Data Initiative Forum III on Population Health ManagementWill Explain How Providers Can Use Automation Tools to Improve Care Management and Coordination Across Their Patient Population (June 05, 2012)
DALLAS, TX -- (Marketwire) -- 06/05/12 -- For the second year in a row, Phytel has been invited to present its provider-led population health management solutions during the Health Data Initiative Forum III, to be held June 5 and 6 at the Walter E. Washington Convention Center in Washington, D.C.
Phytel will deliver a presentation on how to structure and implement a population health management program at 4:35 p.m. on Tuesday, June 5 during the Apps Demos in the convention center's main auditorium.
The Apps Demos are designed to help attendees learn how the newest health IT apps and products focused on healthcare providers, consumers and communities can meet their organizations' needs.
