Phytel Showcased at Health Data Initiative Forum III on Population Health Management

Will Explain How Providers Can Use Automation Tools to Improve Care Management and Coordination Across Their Patient Population (June 05, 2012)

DALLAS, TX -- (Marketwire) -- 06/05/12 -- For the second year in a row, Phytel has been invited to present its provider-led population health management solutions during the Health Data Initiative Forum III, to be held June 5 and 6 at the Walter E. Washington Convention Center in Washington, D.C.

Phytel will deliver a presentation on how to structure and implement a population health management program at 4:35 p.m. on Tuesday, June 5 during the Apps Demos in the convention center's main auditorium.

The Apps Demos are designed to help attendees learn how the newest health IT apps and products focused on healthcare providers, consumers and communities can meet their organizations' needs. Presenters will demonstrate their latest products and answer questions from attendees. Facilitators include Bob Higgins, M.B.A., general partner, Highland Capital Partners, and Wil Yu, director of innovations at the Office of the National Coordinator for Health IT (ONC).

Phytel's next generation of automated care management solutions on its integrated Atmosphere™ platform enable providers to proactively improve chronic and preventive care across their populations. These scalable, Software-as-a-Service automation tools together comprise the first comprehensive, integrated population health management solution designed for providers. Phytel's suite of services enables care teams to deliver appropriate care to all of their patients, regardless of care setting, and to engage patients in their own care. Phytel uniquely combines automated interventions with analytic reports to track and measure the overall effectiveness of quality improvement initiatives.

Also known as "Health Datapalooza," the Health Data Initiative Forum is a public-private collaboration that encourages innovators to utilize health data to develop applications that raise awareness of health system performance and spark community action to improve health. The 2012 Forum is the third annual event and is expected to be the largest yet.

The Health Data Consortium, which sponsors Health Datapalooza, includes the U.S. Department of Health and Human Services, the Institute of Medicine, the California Healthcare Foundation, the Lucille Packard Foundation for Children's Health, the Mayo Clinic Center for Innovation, the National Association of Counties, the Robert Wood Johnson Foundation, and several other organizations.

"We're honored to be among the technology companies invited to present at this year's Health Data Initiative Forum," said Patrick Flynn, chief technology officer of Phytel. "Our population health management solutions define what this event is all about -- using the latest technologies to improve the quality and lower the cost of healthcare. We hope our presentation will contribute to a better understanding of how an automated, technology-based approach can transform our healthcare delivery system."
